One of the many great things about our walk-in urgent care center is that in addition to providing high-quality care for your urgent care needs, we also have certified USCIS civil surgeons on staff who can provide immigration physical examinations for those applying for permanent residency in the United States. This type of medical examination includes bloodwork; vaccinations; and x-ray services.

What do you need to bring with you for your Immigration Physical?

  • Your passport and all vaccination records
  • I-693 Form. You can either print out the form provided for you here or wait until your visit and we will provide you with a form to fill out.
  • Co-pay. Most major health insurance plans do not cover an immigration physical exam. Please be sure to check with your insurance on whether or not this is covered. If it is not covered, we offer affordable self-pay options that won’t break the bank!

What can I expect from AFC Urgent Care during my Immigration Physical exam?

  • Once you arrive, we will start with filling out the I-693 Form.
  • We will begin the medical examination. This includes:
    • A complete physical exam
    • Blood tests for sexually transmitted diseases such as syphilis
    • We will check your vaccination records for any missing required vaccinations
    • Required vaccinations are as follows: mumps, measles, polio, tetanus, pertussis, rubella, hepatitis A & B, influenza (the flu), rotavirus and more.
